What is premature ejaculation?

Premature ejaculation (PE) is defined as male sexual dysfunction characterized by
ejaculation which always or nearly always occurs prior to or within about one minute of
vaginal penetration; and inability to delay ejaculation on all or nearly all vaginal
penetrations; and negative personal consequences, such as distress, bother, frustration
and/or the avoidance of sexual intimacy.  PE can be classified into
life-long PE-presence
of the problem from the onset of initial sexual activity and
acquired PE- indicates the
problem has developed after an initial time of unimpaired ejaculatory function.

How common is it?

PE is the most common male sexual dysfunction in the United States.  It is reported that
approximately 35% of men have PE in some studies.

What is the cause of premature ejaculation?

PE has been attributed to psychogenic and organic causes.  Psychogenic causes
include anxiety, depression, stress, unrealistic expectations, history of sexual repression
and lack of confidence.  Organic causes include low brain serotonin levels, erectile
dysfunction, endocrine abnormalities and chronic prostatitis.  It has been suggested that
PE is a hereditary problem in 25% of men.

What are the treatments for premature ejaculation?

There are currently no FDA approved treatments for PE.  All treatments are considered
"off-label" use of the medication and patients are informed about the consequences of
taking medications "off-label."

  • Local topical anesthetic agents (Lidocaine, Prilocaine)
  • On-Demand SSRI (Paxil, Prozac, Zoloft, Lexapro)
  • Daily SSRI
  • Tricyclic antidepressants (Clomipramine)
  • Tramadol (Ultram)
  • PDE5 inhibitors (Viagra, Cialis, Levitra)
  • Intracavernosal Injections
  • Combination Behavioral Modification/Oral Medication
